This page provides instructions and notes on the use of the Update Database
Information form.
Use this form to update your station capabilities and your compleion
of Emergency Communications courses.
YOU MAY NOT use this form to renew your MARS
license or update personal information such as your name, address, phone
numbers, etc. Use the MARS Memberhip
Form for make those types of changes.

the Update Database Information Form
(you might want to print this page and have it available for reference
while you complete the form) |

Click the link "Fill Out
the Form" at the end of these instructions. After a few moments,
the form will display on your screen. |
| 2. |
Using your mouse, point and click to the box following the word
CALLSIGN: and then type your MARS Callsign. |
| 3. |
Using your mouse, point and click to the area above "First"
following the word NAME. Enter your First Name. USE
ALL CAPITAL LETTERS. |

Use the TAB key to move to the successive areas
and enter the information requested. If you find it necessary to return
to a previous area, use your mouse and point to the area and then
click the left mouse button.
NOTES ON ENTERING DATA
USE ALL CAPITAL LETTERS for text entries.
When entering your DOB: (date of birth) use
the format of mm/dd/yyyy where mm is the two digit
month number, dd is the two digit day number, and yyyy
is the four digit year. This information will be compared with the
DOB recorded in the database. If the two dates do not agree, the submission
will be considered invalid and will be discarded. |

Use your mouse to point to and click on ALL your Station
Capabilities. |

If you have completed and have certificates of completion for any
of the FEMA Emergency Communications course, click the button in front
of course title and enter the date of course completion in the box
below the course. use the format of mm/dd/yyyy where mm
is the two digit month number, dd is the two digit day number,
and yyyy is the four digit year. |

The ECOMM courses refer to the courses sponsored by ARRL. If you
have completed these courses, click on the box in front of the course(s)
you have completed. |

Carefully read and understand the Privacy Statement, Release
Agreement, and Signature Information. |
| 9. |
If you agree to the statements and agreements and have completed
the form, sign the form by typing the equivalent of your normal signature. |
| 10. |
Click on the SUBMIT button. If all required information has
been entered you will see a confirmation that your request has been
sent. If all required information has not been supplied you will receive
a message informing you of missing data and describing a method of
supplying the missing information. |
